COVID-19 vaccinations for students

The Daviess County Public Schools district is partnering with Owensboro Health to provide COVID-19 vaccinations to students age 16 and older. THIS IS COMPLETELY OPTIONAL. 


Parents/guardians of students age 16 and older have been sent an email with a link to register their child to receive the vaccination. The registration form is also available HERE. Registration deadline is Monday, May 3. Owensboro Health will contact parents/guardians to schedule appointments. 


The vaccination is available only to students age 16 and older by May 7. 

The clinic for the first dose is scheduled for Friday, May 7, with the clinic for the second dose scheduled for Wednesday, May 26. 


Owensboro Health ALSO REQUIRES a release form signed by parent/guardian before the student can receive the COVID-19 vaccine. That form is available HERE - Print and complete this form; student will take the form to the vaccine clinic. 


Parents/guardians should contact their child’s primary care physician with any questions regarding possible risks and reactions.
